RESPONSIBILITIES
If you move please let us know your new address and telephone number as soon
as possible so that we can keep our records up to date. This is very important
in case we need to contact you.
If you change your name please also inform us as soon as possible in order that
we may update our records.
PATIENT CONFIDENTIALITY AND DATA PROTECTION
We ask you for personal information so that you can receive appropriate care
and treatment. This information is recorded on computer and we are registered
under the Data Protection Act. The practice will ensure that patient confidentiality
is maintained at all times by all members of the practice team.
COMMENTS/SUGGESTIONS/COMPLAINTS
The practice operates a complaints procedure in line with NHS regulations and
the practice manager will be pleased to discuss any suggestions, problems or
complaints.
